获得性脑损伤(ABI)后视觉功能障碍发生率高达60%~70%,但因其涉及视觉通路及多模态整合网络的中枢源性损伤,常被常规诊疗漏诊或误诊,严重影响患者整体康复进程与生活质量。为规范ABI相关视觉障碍的临床诊疗,中国医学装备协会眼科专业委员会视觉康复学组组织多学科专家,基于建议、评估、制定和评价的证据分级系统,系统检索国内外相关文献,结合中国临床实践,制定了本共识。此共识首次基于国际功能、残疾和健康分类框架将ABI视觉障碍系统分类为视感觉、视运动、视知觉及视觉-前庭功能障碍四大类别,并确立了从症状量化、基础功能、神经眼动到空间多感官整合的四级评估路径。在此基础上,明确了棱镜、选择性遮盖、精准滤光片及神经视觉训练等核心干预手段的临床适应证与操作规范,并阐述了其基于双通路理论及神经可塑性机制的科学内涵。本共识旨在确立ABI神经视光学诊疗的规范化体系,推动眼科学、康复医学与神经科学的跨学科融合,通过早期个体化精准干预,改善患者视觉信息处理效率与日常生活活动能力,助力其重返社会。
